日本人会クリニックでは、シンガポール国外（日本等）で、

新型コロナワクチンの接種を受けられた方を対象に、

シンガポール政府の予防接種記録システム（NIR）への

登録を受け付けています。

 

外来にて受け付けていますので、下記2点をご持参いただき、

診察時間内にお越しください。

 

<<ご持参いただくもの>>

 

(1) シンガポールのIDカード(EP、S Pass等)、

　　もしくは、シンガポール政府（IPA）レター

 

(2) 接種証明書（QRコード付）

 

費用：＄３５＋GST

 

*ご来院のご予定がお決まりの方は、事前に下記2点の写しを

メールにてお送りいただきましたら、確認・手続きを進め

させていただきます。

 

*お振込みをご希望の場合は、ご来院いただかずに、手続きを

進めることが 可能です。その場合は、メールにて上記2点を

お送りいただき、「お振込みにてお支払い」ご希望の旨、

お書き添えください。

 

*接種証明書にQRコードが付いていない、読み取れない等の

場合は、血清検査（抗体検査）を受けていただき、検査にて、

陽性（抗体があること）が確認されれば、NIRに新型コロナ

ワクチンの接種記録を登録致します。

費用：＄６５＋GST

結果の通知：ご指定の電子メールに、結果を送付いたします。

Post COVID vaccination NIR upload

 

The Japanese Association Clinic Spore is one of the clinic approved by MOH to carry out serology test to confirm COVID vaccination status for those who have received the vaccination outside of Spore.

Upon confirmation of positive serology result, we will upload the vaccination record to the National Immunisation Registry(NIR). After which the authority will update the vaccination record into the Healthhub and Tracetogether app/ token.

We will also email a copy of the serology report to each individual.

 

For those who have a valid QR code on their vaccination, we can upload directly to NIR without serology test. For this process, we accept walk-in or via email with fund transfer.

For email, please send us:

1. Spore ID card/IPA/Passport
2. Certificate showing full vaccination details like the type of COVID vaccine and dates received clearly.
3. Contact details like local address, email and handphone number

 

For those who require the serology test, please bring along:

1. Spore ID card (EP, Spass etc) or IPA or Passport
2. Certificate showing full vaccination details like the type of COVID vaccine and dates received clearly.
